Kentucky WMU seeks to partner with churches in keeping kids safe. An Acknowledgement and Assurance (A&A) Form is required from all attending churches. The A&A Form states that your church chaperones have had background checks completed. Download and complete the A&A and email to: [email protected].

*Background checks required for every adult. 1 male chaperone is needed for every 1-5 boys and 1 female chaperone is needed for every 1-5 girls.
