These forms include authorizations, medical information, and other vital statistics to for the health and safety of your participants. Participation is restricted to those with completed forms. There is a place on this form for participant's to include the email of their group leader/ event coordinator. A copy of their responses will be provided to this email alongside the copy sent to their personal email.

This form requires your written attestation that all adults have completed their background checks. Participation is restricted to groups with this form completed. Form can be found as attached fillable pdf, please fill digitally, save, and return via email to stacy.nall@kybaptist.org.
